History of Mnemosyne

Mnemosyne is a lore book added with The Witch Queen, unlocked by completing each of the four Altars of Reflection branches. It holds eight entries, including Altars of Reflection - Dispatch, Assessment, Control and New Input, "RE: Altars of Reflection - New Input", Conclusion [Draft], Personal Accountability and Conclusion. They are reports by a Hidden agent (Lisbon-13 per Destinypedia/Ishtar Collective) on Deepsight and the Altars in the Witch Queen's throne world.
