History of Monument to Failure

The Monument to Failure is a shrine in the Caldera on Kepler that the Guardian visits during step 2 of Faculty Advisement, an Ikora side quest in The Edge of Fate. The Aionian researcher near the Post Box terminal sends you to it.

Each item on the shrine has a log recording a failure, some personal and some disproven hypotheses. The shrine is described as a vow to learn and try again. The quest continues with Dean Rebecca and the Vex sample collection.

Sources differ on when the quest opens: Destinypedia says after step 9 of The Edge of Fate, while ProGameTalk says after the first two Strangelet quests.
